In India anal fistula constitutes 2per cent of all surgical admissions. A fistula in ano is a track that develops from the inner lining of the anus through the tissues that surround the anal canal. The tract opens into the skin near the anus. When the tract is repeatedly infected due to the bacteria in the stool, pus is formed and tract is closed. This condition is painful and pus is discharged. The tract is formed deep into anus through important muscle known as levator ani .
CAUSES
Long vehicle drives, anal abscess and infected bowel, chronic constipation, irregular bowel habits, untreated fissures and piles are some causative factors. Fistula can also form as complication in the surgery. In some cases, it can be from the birth and a history of IBD (infective bowel disease), diverticulitis, previous radiation therapy, steroid therapy, or HIV infection may suggest a more complex fistula.
SYMPTOMS
Symptoms are usually a purulent discharge and drainage of pus, pus with blood, and/or with stool near the anus, which can irritate the outer tissues causing itching and discomfort. Pain occurs when fistulas become blocked and abscesses recur. Gas may also escape from the fistulous tract.
Is it a dreaded disease?
Yes! Anal Fistula becomes a dreaded disease if treatment is not given on time. The pus inside the fistula can spread further leading to formation of multiple tracts and several openings around the anus internally and externally.
KSHAR SUTRA THERAPY IN FISTULA IN ANO
Kshar is referred to anything corrosive or caustic. Sutra means thread. In Ayurveda, Ksharasutra treatment is done with medicated thread to treat fistula and other similar problems. Kshar Sutra is a para-surgical procedure invented by the father of Surgery Susrutha, who used to treat Nadi vrana (sinus) Bhagandhara (fistula in ano) and arbuda (small benign tumours) etc. Kshar Sutra is a part of Shalya Tantra and the same has to be done by Ayurvedic doctors who have expertise in Kshar Sutra Treatment. The Kshar Sutra therapy in fistula in ano is having many advantages with minimal complications and thus can be considered as preferable method of Treatment for the following reasons – It is technically safe, simple and does not require any special paraphernalia.

It can be carried out in the OPD, as most of the cases do not require any anesthesia also it is considered as cost-effective treatment as compared to others. It does not require hospitalisation and patients can continue with their routine work, except brief rest. The rate of recurrence after the treatment is negligible as compared to other treatments. It is an ideal management for the patients of old age or having respiratory or cardiovascular diseases and or otherwise unfit for Surgery. Post-operative tissue damage and scarring are minimal.
